NEW VOLUNTEERS
INITIAL SIGN UP PROCEDURE
1. You, the prospective volunteer, must complete the Volunteer Application and the Code of
Pastoral Conduct Acknowledgment of Receipt and Review and submit to the St. Francis
Cathedral Rectory Office – c/o Kara Kwiatkowski.
2. In addition, you should set up and attend appointments for:
a. Fingerprinting* (Fingerprint Form)
b. Protecting God’s Children Workshop (Registration Instructions)
Retain copies of the Fingerprinting form, fingerprinting receipt, and Workshop certificate.
*The fingerprinting will initiate a background check by the State Police. This takes anywhere
from 2 weeks to 1 month. This background check can be reimbursed through the St. Francis
Cathedral (SFC) Rectory.
3. Upon completion of the fingerprinting and workshop, complete the two-part form:
“Volunteer Fingerprinting Reimbursement Request”/”Protecting God’s Children Workshop”.
You will need to include a copy of the fingerprinting receipt (if you request reimbursement)
and the Workshop Certificate. Submit form and copies to the St. Francis Cathedral Rectory
Office – c/o Kara Kwiatkowski.
4. When all of the above requirements are met and the SFC Rectory Office is notified by the
State Police of background clearance, a Volunteer ID badge (with a 3 year expiry from the
date of fingerprinting) will be issued.
**Please note that educators who work outside the Diocese of Metuchen school district (for
example, in public schools) will require this additional background check. SFCS does not
have access to background check archives nor updates conducted by schools outside of the
Diocese of Metuchen.
CURRENT/PRIOR VOLUNTEERS
RENEWAL PROCEDURE – Please contact the SFC Rectory Office at 732-548-0100 for a
state issued BLUE form.
1. Current/Expired Volunteers will need to complete the Name Check Only Form (Blue Form)
and submit to the SFC Rectory Office – c/o Kara Kwiatkowski. PLEASE DO NOT INCLUDE
MONEY WITH THIS FORM.
2. The form will be mailed out to the New Jersey State Police with a check from the Rectory
.
3. When the SFC Rectory Office is notified by the State Police of background clearance, a new
Volunteer ID badge (with a revised 3 year expiry from the date of the name re-check) will be
issued.
As a reminder, this background re-check takes anywhere from 2 weeks to 1 month to
complete. Therefore, it is advised to submit this form approximately 1 month before your
badge expiration
